正常成人每天合成的免疫球蛋白为2～5g[1]。但是在受到感染以后，其的合成量可增加7倍左右[2]。因此，对Ig的测定可以作为检测多种疾病的依据，同时在临床上，还将其作为临床诊断和观察以及预后的重要观察指标。然而，对血清Ig的检测结果也同时受到多种因素的影响，比如检测仪器；标本处理；操作技术；实验试剂等。在同一实验室的不同批次或不同实验室中对其进行检测，也会出现一定的差异。但是，在国内对血清标本的放置时间和保存温度对IG的检测结果存在的影响，报道的非常少。本文采用全自动生化检测仪器对不同温度和时间对血清进行保存，并对Ig进行检测，主要是探讨临床上保存血清的最佳条件。以下是详细报道。

1 资料和方法

1.1 一般资料 我校在2012年6月～2013年10月期间，一共选择了200名健康的体检学生。都是男性，年龄在22～30岁之间，平均年龄为（27.5±2.0）岁。他们都没有感染性和系统性的疾病，同时没有出现肝肾和心血管等疾病。

1.2 方法

使用干燥的试管对他们进行静脉抽血，大约为5ml。在室温下放置1h，使用3500rpm进行离心10min，最后去最上层的血清，分开装到5根试管中。分别放置到室温和4℃以及-20℃的环境中进行保存。在室温下进行保存的血清，在保存2h后，对其进行检测IgA、IgG、IgM。保存在4℃和-20℃的血清，分别在第3d和第6d进行检测。IgA、IgG、IgM的定量测定试剂盒由上海科华生物工程股份有限公司生产。用Hitachi 7180全自动生化分析仪（日立公司生产）进行免疫比浊法测定，所有操作均严格按照试剂盒说明书进行[3]。

1.3 统计学方法 对所有的数据都使用均数±标准差（x±s）来进行表示[4]。并使用SAS15.0软件进行统计和分析。并采X2检验。差异显著具有统计学意义（P

免疫比浊法是目前临床上测定血清IgA、IgG和IgM的主要方法，包括透射比浊法和散射比浊法。两种方法的测定原理和仪器不同，其结果有所差异。透射比浊法主要检测抗原抗体复合物所形成的浊度，用全自动生化分析仪检测；散射比浊法是从不同角度测量抗原抗体复合物微粒的散射光强度和浊度的变化，用蛋白分析仪检测。一般认为散射比浊法的灵敏度高、重复性好，但国内研究显示散射比浊法和透射比浊法准确性、精密度均比较好。但散射和浊法和透射比浊法测定Ig的结果，发现偏差均在可接受范围，变异系数均小于3%，说明二者均为IgG、IgA、IgM的可靠测定方法。全自动生化分析仪的应用，使Ig的检测速度大为加快。如果使用性能较好的全自动生化分析仪及相应的配套试剂，就能保证IgA、IgG和IgM透射比浊法测定结果与散射比浊法一致。日立7180全自动生化分析仪具有处理能力大、测试速度高、标本用量小等优点，在精密度、线性、准确度等方面性能良好。

血清标本的分离与保存、检测仪器和试剂、操作技术等因素均对Ig检测结果有影响，在检测过程中必须做好各个环节的质量保证措施，减少或消除检验误差。用全自动生化分析仪测定血清样品，新鲜标本较好。由于免疫比浊法测定的是溶液浊度，故脂血、溶血标本对测定结果影响很大。适宜的温度可加快抗原与抗体结合，从而加速了浊度反应。在0～56℃之间，温度越低，反应越慢，但反应生成的抗原抗体复合物结合致密、牢固，易于观察；温度越高，反应越快，但反应生成的抗原抗体复合物不稳定，易解离。温度过高（>56℃）可能引起抗血清球蛋白变性及某些血清球蛋白破坏。因此，多数抗原抗体反应以37℃为宜。

经过实验表明，在4℃下对血清进行保存3d，IgA的含量出现了明显性的变化。然而IgG在6d时也出现了很明显的升高情况[6]。但是IgM却没有出现变化。在-20℃条件下对血清进行保存3～6d时，IgA和IgG以及IgM的含量都没有出现明显性的变化。在4℃时对血清进行保存时，IgA和IgG的含量升高，然而IgM的含量却没有变化，主要是因为血清中的IgA、IgG大多是单体。IgA和IgG的分子肽链相对于IgM来说，更容易被血清中的蛋白酶水解掉各种的片段。然而这些被水解的片段又会与试剂中的相应的抗体进行结合。研究结果表明：在4℃下保存6d的血清不适合进行Ig的检测。但是在-20℃下保存的血清却可以进行Ig的检测。
